If you are a physician and would like to make a referral please complete the online referral form. In addition to the online referral form state law requires a completed Physical Examination Therapy Referral Form. Please download and complete the form and fax to our offices at 210.826.7887.
Need More Information about ECI and the services provided?
If you are a physician who provides care to children from birth to age three it's important that you know about and refer families (as required by state law) to local ECI programs if their child has a delay or disability. ECI Publications are available at http://www.dars.state.tx.us/ecis or you can download this order form and fax it to us at 210-826-7887 for small orders. If your office would like an ECI presentation, please complete the online form request and our Public Outreach Coordinator will contact you to deliver information or set up a time to visit your office.
