If you are a physician and would like to make a referral please complete the online referral form. In addition to the online referral form state law requires a completed Physical Examination Therapy Referral Form. Please download and complete the form and fax to our offices at 210.826.7887.
Need More Information about ECI and the services provided?
If you are a phsician who provides care to children from birth to age three it's important that you know about and refer families (as required by state law) to local ECI programs if their child has a delay or disability. If you would like more information about ECI services or if you would like someone to come speak to your staff about what ECI is, please complete the online form request and our Outreach Coordinator will contact you to deliver infomration or setup a time to visit your office.
